Requirements for New Passport with Change of Name (Minor)

1. Duly filled application Form 1D.
2. Two recent passport size photographs (size 35mm X 45mm) with plain background
3. Original passport of the applicant
4. Applicant has to come in person.

Fees: KWD 7.650               

Note: Those born in Kuwait should first change their name on the birth card   issued by the Ministry of Health prior to change of name on passport. For the same, a no-objection certificate can be obtained from the Embassy for obtaining a new birth card with new name from the Ministry of Health (Fees for no-objection certificate is KWD 6.250)
